Significant Occurrence Procedure
This procedure supports the Health and Hygiene and Safety policies
A “Significant Occurrence Record” form is completed in the case of a major incident, including those that are reportable to RIDDOR. Major occurrences include:-
On the Significant Occurrence Record form the following is recorded: the date and time of the incident; nature of the event; who was affected; what was done about it; if it was reported to the police, and if so a crime number; any follow up, including if an insurance claim is made.
The record is normally completed by the manager or deputy manager
In the unlikely event of a terrorist attack, we follow the advice of the emergency services with regard to evacuation, medical aid and contacting children's families. Our standard Fire Safety and Emergency Evacuation Policy will be followed. The incident is recorded when the threat is averted.
In the unlikely event of a child or adult dying on the premises the emergency services will be called and the advice of these services followed.
Completed “Significant Occurrence Record” forms are stored in a Occurrence Folder in the Manager’s secure cupboard.
The procedure was adopted by Laleham Church Playgroup at a meeting on 26th November 2019
